Joe...That would be a Hammond model 812 Primary 150 through 600
Secondary at 12K to 48K...Drives a pair of 
Modulators to full 100% with about a 0 DBM...Work great..These are easy
to find but about $75

As always I am looking for something hard to find. Would like to find
two alike audio transformers low wattage  with around 10,000 ohm
impedence center tapped primary and a 500 or 600 ohm impedence center
tapped secondary.
